The Pair of Eyeballs is a Hardmode pet-summoning item that spawns Rez and Spaz, miniature versions of The Twins in their first form. The Pair of Eyeballs has a 1/4 (25%) chance of being dropped by The Twins in Master Mode.

Trivia

Unlike the Suspicious Eye, Rez and Spaz do not change phases. They are also smaller than the Suspicious Eye.
The item tooltip is a reference to the phrase two heads are better than one.
The buff tooltip may be a reference to a commercial by the American contact lens retailer 1-800 Contacts.
The Pair of Eyeballs and Resplendent Dessert are the only pet-summoning items that spawn two pets instead of one.
- However, the Pair of Eyeballs is the only one that is not crafted.

The names Rez and Spaz are abbreviated forms of The Twins' individual names, Retinazer and Spazmatism.
